<p>Our mission at <em>Apicha Community Health Center</em> is to improve the health of our community and to increase access to comprehensive primary care, preventive health services, mental health, and supportive services.&nbsp; We are committed to excellence and to providing culturally competent services that enhance the quality of life.&nbsp; We advocate for and provide a welcoming environment for underserved and vulnerable people, especially Asians and Pacific Islanders, the LGBT Community and individuals living with and affected by HIV/AIDS.</p> <p>&nbsp;</p> <p>We are currently seeking a <strong>Nurse Practitioner </strong>to work at our 400 Broadway location.</p> <p>&nbsp;</p> <p><strong>Position Summary:</strong></p> <p>The <strong>Nurse Practitioner</strong> will provide comprehensive care to families as well as engage in health promotion activities. They will provide care to a panel of adult patients on a continuing basis including but not limited to; Evaluation and management of acute and chronic medical conditions; Initial assessment and management of mental health conditions e.g. depression and anxiety disorders; Ordering and analyzing necessary diagnostic tests using high value care principles and EBM to inform clinical decision-making; Prescribing appropriate treatment and/or medication in an evidence-based manner and within the scope of licensure; Advising patients on treatment recommendations, lifestyle modifications, and age-appropriate preventive screenings; Referring patients for consultant services when necessary.</p> <p>&nbsp;</p> <p><strong>Qualifications:</strong></p> <p>Qualified applicants must be certified as a Family Nurse Practitioner. A master&rsquo;s degree is required. Successful completion of an accredited nurse practitioner program is required. The NP will have a current and valid NYS license, DEA certificate, BLS, and Infection control certificate. AAHIVM certification and/or LGBT health certification is a plus.</p>
